Xanthogranulomatose choleccistitis imitating vesicular cancer. Case report

Abstract

Xanthogranulomatous cholecystitis is a rare disease that presents clinically with right upper quadrant pain, the most important differential diagnoses of which are chronic cholecystitis and gallbladder cancer. In imaging studies, thickening of the gallbladder wall with irregularities in its trajectory is observed, the treatment is laparoscopic cholecystectomy or open cholecystectomy, the diagnosis being histopathological confirmation. In addition, it is associated with the presence of intra-abdominal adhesions that can make it difficult to extract and generate surgical complications. We present the case of a 45-year-old male patient with clinical abdominal pain and sonographic study of suspected gallbladder cancer. After complementary studies, a cholecystectomy was performed, finding in the histopathological report the presence of Rokitanski Aschoff sinuses and granulomas made up of foreign body-type giant cells and foamy histiocytes characteristic of xanthogrenulomatous cholecystitis. The patient was discharged with adequate progress and without complications.
